AU details, Vash died in the fall. Knives has spent 150 years working on killing any humans he doesn't feel deserves to live. Shockingly some do qualify, like Rem, and he's decided humans may be able to be domesticated with rigorous culling and selective matching of low-vice people. A lot of people have died in the process. Unsuitably aggressive humans that can still be "tamed" are kept on as handy executioners. Unusually durable or desert adapted humans who need less to survive are kept even if aggressive with the intent of eventually isolating what he wants. This AU is more cyberpunk flavored, so cybernetics are far more common than genetic engineering.
Knives is reluctant to wipe out entire towns because this may accidentally kill humans with desirable traits he wants for his project, and it's hard for them to learn if they're ALL dead. Even then without anyone to stop him, the human population of Gunsmoke is probably 1/3 of what it is in Maximum pre-ark and steadily shrinking. He doesn't really see humans as people, or he wouldn't be treating them like livestock.
